Commonwealth Home Support Program (CHSP)
Centre for Participation is a not-for-profit provider of the federal government’s Commonwealth Home Support Programme. This program provides entry-level home care and support for people aged 65 years and over (or 50 years and over for Aboriginal and Torres Strait Islander people), who need some help to live independently at home. At the Centre we can provide:
Transport – If you are our CHSP client and have transport with My Aged Care we can provide you with Taxi vouchers for local trips in Horsham or if needed a volunteer to transport you to the shop for shopping assistance. If you need to get to the hospital or an appointment, we will organise a volunteer driver to get you there (maximum journey distance is to Ballarat). If you just want to attend social activities with your friends/family and are finding transportation difficult give us a call 5382 5607
For all transport services a client contribution is required. Depending on your My Aged Care status the following charges apply.

Social Support – If you are our CHSP client and have Social Support with My Age Care we can provide you with individual or if you prefer group social support activities.
Individual Social Support – you tell us what you need (trip to the shops, go to the cinema, go for a cuppa, companion to go to a medical appointment etc) and we will endeavour to find a volunteer to assist you.
Group Social Support – want to join a group for a Bus outing. We run regular small group trips on our bus to various areas around our district. One week you could be going to the Military Museum in Horsham, the next week you might have a picnic in Halls Gap or maybe a journey around the Silo trail.

8 Aug 2024 – 7.30 am Bus Outing leaving from Centre for Participation to Wendouree Shopping Centre returning to Centre for Participation around 4 pm! Cost $10.00. Book by 1st Aug 2024
15 Aug 2024 – 1.00 pm Bus Outing leaving from Centre for Participation to Grampians Olive Company (base of Mt Zero). Return time around 4.00pm. Cost $2. Book by 8 Aug 2024
22 Aug 2024 – Bus Outing leaving from Centre for Participation to Pomonal/Halls Gap/Lake Bellfield and Halls Gap visitors centre returning to Centre for Participation 1.00. Cost $2.00. Book by 15 Aug 2024
Men’s Only Bus Outings – CHSP transport and social support clients
13 Aug 2024 – 9.00 am Men’s Outing from Centre of Participation to Kookas Country Kookies in Donald. Return time around 1.00pm. Cost $2. Book by 6 Aug 2024.
20 Aug 2024 – 1.00 pm Men’s Outing leaving from Centre for Participation to Grampians Olive Company (base of Mt Zero). Return time around 4.00pm. Cost $2. Book by 16 Aug 2024. Book by 13 Aug 2024
27 Aug 2024 – 9.30 am Men’s Outing from Centre of Participation to Seppelt Winery in Great Western – underground cellars tour. Note walking tour approx. 600m with one flight of 30 stairs. Return time around 1.30pm. Cost $22. Book by 20 Aug 2024.
Specialised Support Services – each month we have a morning tea (on a Wednesday) for any member of the public who is interested in attending. During the morning there will be a guest speaker providing information on topics of Specialised Supports including but not limited to Continence, Dementia, Hearing and Vision. We can also advise people how to link into My Aged Care, provide Client Advocacy and any information required regarding Home Care Packages and the providers in the region.
