Our individualised services

Our social support services aim to keep you living at home and in your community as independently as possible.  First we find out the things that are important to you and what you want to achieve. Then together we help put in place the right solutions for you, you are at the center of service provision.  We work with you to help you do as much as you can for yourself to help maintain your independence.

We are able to put in place services that reflect your personality, needs and goals. Our services cover the Wimmera region and we are able to provide assistance where ever you are.

 

General services

We help you do as much as you can for yourself to help maintain your independence with programs and activities aimed at reducing social isolation, improve confidence and overall wellbeing.

Town services

Our shopping assistance service covers a volunteer who will pick you up from your door and take you into town to do grocery shopping, banking and other essential stops.

Friendly Visiting

Friendly Visiting links volunteers with elderly residents living in their own own home.  The program aims to create ongoing friendships and to reduce social isolation. Friendly Visiting enables elderly residents to retain their independence while improving social contact and connectedness.  The program improves the quality of life for residents and requires someone who enjoys providing support and making new friendships.

Community Visiting

Community Visiting provides social interaction and friendship for people living in aged care residential facilities. We match volunteers with a resident, aiming to find common interests that will spark an ongoing friendship.  All volunteers receive training and ongoing support.

A Community Visitor enhances the quality of life of a resident encouraging social interaction.  Volunteers become a vital link for a resident to maintain community connectedness.   The program requires a commitment of at least an hour a fortnight.

Carer Respite

We deliver respite either separately or as part of a flexible, responsive, integrated package of services. We actively consider how to support the person and their carer in maintaining or strengthening the care relationship. Our respite services can be provided at home, in a community venue or in the general community, during weekdays and weekends, on a regular basis, episodically or intermittently as needed.

Social Support Outings and Activities

Centre for Participation helps connect people with an interest group/s or social opportunities; helping clients to get to places they want to go. We also provide enjoyable age-appropriate and meaningful social outings aimed at creating or extending social connections.

Know Your Gizmo

Volunteers support service users to understand how to operate their ‘gizmos’ such as phones, tablets and internet / apps to enable them to connect, in order to maximise their social connecting opportunities.

Memory Lane Cafe

Memory Lane Cafè are held in Hindmarsh Shire, Horsham Rural City and West Wimmera Shire for people with memory loss and their carers. This is an enjoyable outing with the chance to meet people, enjoy some entertainment, find out information, share stories and experiences.

All this in a relaxed, safe and supportive atmosphere with a scrumptious FREE afternoon tea.
